This involves looking\nfor and collecting all the information, descriptive as well as explicative,\nlikely to clarify the causes of an unsafe event. <\/p>\n\n\n\n<p>All\nthe information, proof and declarations available and linked to the elements in\na situation which have led to the accident or incident, are evaluated, so as\nto\ncheck what can be considered as proof or not. <\/p>\n\n\n\n<p>The most probable scenario is then established. The careful analysis of a safety management system allows possible failures and\/or inadequacies to be revealed.<\/p>\n\n\n\n<p>An accident or incident is generally\ncaused by human error: stopping at this finding does not allow the necessary\nlessons to be learned, does not allow the error to be avoided in the future,\ndoes not reduce the impact, etc. <\/p>\n\n\n\n<p>This is why we look for the direct causes and the\ncontributing factors that allowed the accident to happen. <\/p>\n\n\n\n<p>Our findings cannot be interpreted as\nattributing or determining civil or criminal liability. The questions and\nreasonings are laid out in line with the IU\u2019s sole objective: to promote safety\nby encouraging stakeholders to consult with each other, by continuing to invest\nin and be involved in improving safety on the Belgian railway network. <\/p>\n\n\n\n<p>In the context of our investigations, we\nensure that all people and organisations are treated with respect, courtesy,\nequality and discretion.<\/p>\n\n\n\n<p>The investigations are conducted in a\nsystematic, exhaustive and impartial manner.<\/p>\n\n\n\n<p><!--StartFragment--><\/p>\n\n\n\n<p><!--EndFragment--><\/p>\n\n\n\n<p><\/p>\n<\/div><\/div><\/div><\/div><\/div><\/div>\n\n\n\n<div class=\"wp-block-ugb-accordion ugb-accordion ugb-ec9fe6b ugb-accordion--v2 ugb-accordion--design-basic ugb-accordion--single-open ugb-main-block\" aria-expanded=\"false\" id=\"\"><style>.ugb-ec9fe6b .ugb-accordion__heading{background-color:#35a8e0 !important}.ugb-ec9fe6b .ugb-accordion__heading:before{background-color:#35a8e0 !important}.ugb-ec9fe6b .ugb-accordion__title{color:#ffffff}.ugb-ec9fe6b .ugb-accordion__arrow{fill:#ffffff}<\/style><div class=\"ugb-inner-block\"><div class=\"ugb-block-content\"><div class=\"ugb-accordion__item\"><div class=\"ugb-accordion__heading\" role=\"button\" tabindex=\"0\"><h4 class=\"ugb-accordion__title\" role=\"heading\" aria-level=\"3\">The investigation report<\/h4><svg viewbox=\"0 0 20 20\" xmlns=\"http:\/\/www.w3.org\/2000\/svg\" class=\"ugb-accordion__arrow\" width=\"20\" height=\"20\"><path d=\"M16.7 3.3L10 10 3.3 3.4 0 6.7l10 10v-.1l10-9.9z\"><\/path><\/svg><\/div><div class=\"ugb-accordion__content\" role=\"region\"><div class=\"ugb-accordion__content-inner\">\n<p>Their\ngoal is to encourage the circulation of knowledge acquired in\nthe course of\ndifferent analyses.\n<\/p>\n\n\n\n<p>The\npreliminary reports are sent to the actors concerned, so as\nto\nallow them to get to know the analyses and to provide their comments. The goal\nis not to alter the content of the report but to add any necessary details. <\/p>\n\n\n\n<p>The\nconclusions and recommendations are a part of the draft final report sent to\nthe actors concerned.
